I painted the concrete floor in my basement and now the paint is coming up. I want to repaint but want to do it right. Any suggestions on the correct method to do it right this time?

We have concrete walls in our garage that have rough spots on them. I'm not sure how to get rid of the rough spots or should I just paint right over them? What kind of paint would work best? Thanks!
